Description Under direction, to assess, plan, implement and evaluate education and to provide other health services for a specific patient population; to function as a consultant and educator to staff. This position requires prior experience in End-Stage Renal Disease (ESRD) care, including coordination of dialysis vascular access, knowledge of hemodialysis treatment processes, and demonstrated experience with Centers for Medicare & Medicaid Services (CMS) Conditions for Coverage (42 CFR 494), Quality Assurance and Performance Improvement (QAPI) programs, and dialysis Plan of Care requirements. Candidates must have day-one proficiency in dialysis-specific regulatory standards, quality metrics, and interdisciplinary care coordination.
