Licensed Vocational Nurse II - Nurse Support Teams

ABOUT THE POSITION

The County of Riverside - Riverside University Health System (RUHS) - Medical Center is seeking an experienced Licensed Vocational Nurse II to join its Nurse Support team (Diabetes Program) located in Moreno Valley.
Under supervision and direction of a registered nurse, nurse practitioner, physician assistant, or physician, the incumbent will be responsible for performing reviewing medical records and abstracting diabetes-related clinical data in alignment with established criteria and program measures. The position retrieves, compiles, and summarizes statistical information to support performance improvement activities, regulatory reporting, committee review, research, and other hospital needs. It maintains accurate and detailed records within the database, assists in preparing reports, charts, data summaries, and meeting materials, and communicates effectively with nursing staff, medical staff, administration, medical records, and other departments. Additionally, the role supports performance improvement efforts by identifying data trends, documentation gaps, workflow opportunities, and education needs, as well as assisting with unit-based diabetes education. The position upholds data integrity and confidentiality in compliance with HIPAA and organizational standards while maintaining a clean, organized, and uncluttered work environment; as well as, performing nursing duties within the scope of the Nurse Practice Act for vocational nursing in the care of patients in the Acute Care or Emergency Department; and perform other related duties as required.

The Inpatient Diabetes Program supports hospitalized patients with diabetes and hyperglycemia through clinical consultation, education, quality improvement, and data-driven performance monitoring. The team collaborates closely with nursing staff, providers, pharmacy, nutrition services, case management, and hospital leadership to promote safe, evidence-based diabetes care. The program focuses on improving inpatient glycemic management and maintaining readiness for disease-specific program review. This role is an important part of the Diabetes Team and supports the program’s ability to collect, validate, analyze, and report data that drives patient safety, performance improvement, staff education, and regulatory readiness. The position also assists with translating data trends and identified opportunities into practical unit-based education, staff feedback, in-services, and education support.

The most competitive candidates will possess the following professional experience:

• Acute care hospital experience
• Experience with diabetes care
• Experience with chart review, data abstraction, clinical documentation review, quality improvement, or registry/database maintenance
• Familiarity with Epic
• Proficiency with Microsoft Excel, Word, and PowerPoint
• Experience preparing reports, charts, or summaries
• Experience assisting with staff education or quality improvement education
• Bilingual

The ideal candidate will be analytical and comfortable working with clinical data, electronic medical records, and unit-based education. They should demonstrate strong attention to detail, excellent organization and time-management skills, and the ability to meet deadlines. Effective communication and the ability to collaborate with multidisciplinary teams are also essential.

This role is more than clerical data entry and requires the ability to review patient records, abstract information accurately, identify documentation gaps, support reporting needs, and communicate effectively with clinical and leadership teams.
Required valid licenses and certificates:

• California Vocational Nurse's License issued by the California Board of Vocational Nurse and Psychiatric Technician Examiners
• Basic Life Support (BLS) certification issued by the American Heart Association

Work Schedule: 
Work hours will be 4/10 shift working 7:00am-5:30pm; hours are subject to change based on departmental needs.

EXAMPLES OF ESSENTIAL DUTIES

•Provide nursing care to patients within the scope of the Nurse Practice Act for vocational nursing; prepare patients for physicians, registered nurse, physician assistants, and nurse practitioners, and care for patients during treatment; administer medications using subcutaneous, intramuscularly, intradermal, subdermal, and/or oral methods.
•Take temperature, pulse, respiration and blood pressure of patients as necessary; apply and change dressings and cares for wounds; may collect specimens for laboratory analysis; assist physicians, registered nurses, and mid-level providers in performing examinations, tests, respiratory care and other treatments including minor surgical procedures, PAP smears and colposcopies.
•Perform venipuncture, injections of medications including biological immunizations for pediatrics and adults, including the provision of Tuberculin skin tests, Coccidioidin skin tests, and Histoplasmin skin tests in the course of a tuberculosis control program.
•Perform documentation as required within the scope of the Nurse Practice Act for vocational nursing, and established rules of charting; chart treatments and medications and maintain pertinent medical records; collect data from patients to ensure scheduling with appropriate services; maintain clinical records, reports and charge tickets; review charts to determine type of treatment the patient may require and assure proper clinic scheduling and continuity of care.
•Counsel, advise and educate patients regarding proper health care methods; set clinic appointments per established policies and procedures; refer patients for specialty care to outside agencies utilizing knowledge of payor source to ensure appropriate referrals to outside agencies and programs.
•Assist physicians, registered nurses, and mid-level providers in the administration of special treatments, examinations, and intravenous therapy; chart treatments and medications and maintain pertinent medical records according to established practice by the Nurse Practice Act for vocational nursing, policies, and procedures.
•Assist physicians, registered nurses, and mid-level providers in the administration of medical records; assist in the data collection, preparation, and implementation of patient care plans; participate in patient education and rehabilitation programs and document data appropriately.
•Observe patients' condition and behavior, and report significant changes to the supervisor.
•May act under supervision and direction of a physician, registered nurse or mid-level provider as a nursing leader over health services assistants, certified medical assistants, office assistants and other LVNs, as assigned.
