ORGANIZATION OVERVIEW
Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to those transitioning from or at risk of homelessness or institutionalization. We develop, own and manage multi-family supportive housing and licensed residential care homes. Additionally, we implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans’ services, and health care sectors.

We have offices statewide with a staff of passionate people. While we are growing rapidly, we are also selective: we’re seeking inspired, talented people who want to be part of a team affecting profound change and who have fun doing it.
PROGRAM DESCRIPTION
IE Rise is a new and innovative program that is a collaborative effort between the Inland Empire Health Plan (IEHP) and Brilliant Corners. The IE Rise program serves IEHP members with chronic medical and/or behavioral health conditions who are currently residing in an institutional setting unnecessarily. The goal of the program is to transition those individuals to a lower level of care, either to independent living or a residential care facility, and provide the appropriate resources and support to improve their health and overall wellbeing. Brilliant Corners provides intensive case management services to all clients in the IE Rise program, as well as a rental subsidy and housing services to those clients who are able to live independently in the community.
